Call for Letters of Interest—Rural Geriatric Community Capacity Building Partnerships

The University of Wisconsin-Madison School of Nursing and the Wisconsin Office of Rural Health seek to partner with three rural Wisconsin communities to plan for improved support of older adults and their caregivers by integrating novel care resources in their communities.

Selected communities will receive up to $15,000 and technical assistance from UW project staff to assess older adult and family caregiver needs in their communities, identify resources or programs to meet needs, and develop community action plans related to older adult well being. The grant activity period will run from February 1, 2018 to November 30, 2018. Grant awards are contingent on available funding.
