A progress report from the Partnership for Value-based Coordinated Care

WPHA-WALHDAB’s mid-term legislative priority

 

Program summary:

Join us for an update on the progress made since the Initial business case for coordinated team-based care was released in August 2016. The Partnership for Value-based Coordinated Care has made significant progress in formalizing this partnership, updating the business case, and working toward identifying the essential elements of coordinated team-based care.

Speaker bios:

Kristen Grimes

Director of Strategic Partnerships, Children’s Health Alliance of Wisconsin

President-elect, Wisconsin Public Health Association

 

Kristen Grimes has worked at Children’s Health Alliance of Wisconsin since 2001. Her current work includes management of the Wisconsin Asthma Coalition, implementation and oversight of the Wisconsin Asthma Plan, and facilitating the Partnership for Value-based Coordinated Care. Her work with the partnership includes the development of a business case for coordinated team-based care.

 

Ms. Grimes is the Wisconsin Public Health Association (WPHA) president-elect and member of the Joint WPHA-Wisconsin Association of Local Health Departments and Boards public affairs committee. She also serves as a health promotion policy expert for the Society for Public Health Education (SOPHE) and serves on the SOPHE advocacy committee. 

 

Ms. Grimes received her bachelor’s degree in community health education from the University of Wisconsin La Crosse in 1999. She earned her master’s degree in organizational management from the University of Phoenix in 2004. Ms. Grimes has been a certified health education specialist (CHES) since 1999 and received her master CHES in 2011.

 

 

Sarah VandenHeuvel

Project Coordinator- Wisconsin Asthma Coalition

Children’s Health Alliance of Wisconsin

 

Sarah VandenHeuvel is a project coordinator at the Children’s Health Alliance of Wisconsin (Alliance). Her primary job responsibilities are to coordinate the activities of the Wisconsin Asthma Coalition (WAC), which include implementing the WAC’s school and child care walkthrough programs. She joined the Alliance in May 2016.

 

Ms. VandenHeuvel’s previous experience includes chronic disease advocacy in Washington, D.C. and community health research at a local research center. She graduated with a bachelor’s of arts degree in sociology from St. Norbert College and is currently pursuing her master’s of public health degree from the Medical College of Wisconsin
