The WPHA Awards Committee is currently seeking nominations for awards to be presented at the 2008 Annual Public Health Conference, July 23 and 24 in Madison. The Awards Committee is now accepting nominations throughout the year. However, for a nomination to be considered for the 2008 Annual Conference, the deadline for submission is Friday, April 18, 2008. Nominations received after that date may be re-submitted for the next annual conference.

The Awards process is the single most important means that WPHA has for recognizing those who have made significant contributions to public health in Wisconsin. We need your help to identify candidates for the following award categories.

  • Excellence in Health Promotion and Disease Prevention
  • Presidential Citation
  • WPHA Member of the Year Award
  • New Public Health Worker of the Year
  • Distinguished Service to Public Health
  • Special Achievement of a Public Health Partnership
  • Excellence in Public Health Research
  • Legislator of the Year
  • Media Award

Please review the WPHA Awards Table for more details on the purpose and criteria for each award. Then complete the nomination form and return it to the Awards Committee Chairperson. You can link to the awards table and nomination form at the bottom of this page.

Nominations and questions regarding the awards process or criteria for selection may be directed to Eric Krawczyk, Awards Committee Chairperson, at ekrawczy@oneidanation.org or phone at 920-869-4812.
