The WPHA Awards Committee is currently seeking nominations for awards to be presented at the 2012 Annual Public Health Conference, May 21-23 at the Kalahari Resort in Wisconsin Dells.  The Awards Committee accepts nominations throughout the year; however, for a nomination to be considered for the 2012 Annual Conference, the deadline for submission is Friday, April 13, 2012.  Nominations received after that date may be re-submitted for the next annual conference.


The Awards process is the single most important means that WPHA has for recognizing those who have made significant contributions to public health in Wisconsin.  We need your help to identify candidates for the following award categories.
 

  • Excellence in Health Promotion and Disease Prevention
  • WPHA Member of the Year Award
  • New Public Health Worker of the Year
  • Distinguished Service to Public Health
  • Special Achievement of a Public Health Partnership
  • Excellence in Public Health Research
  • Legislator of the Year
  • Media Award
  • Carol Graham Lifetime Achievement Award

Complete and submit the nomination form, according to the directions on the form, by April 13, 2012.
